Contrast agents
A contrast agent does not make a picture. It changes how a tissue or a vessel appears in a picture that would have been taken anyway.
A contrast agent is often described as a dye, but the word dye undersells what it actually does. A contrast agent does not create the picture itself. The scanner, whether it is the CT scan or the MRI scan machine, produces the image in exactly the way it would have done anyway. What the agent changes is how a particular tissue, fluid or blood vessel appears within that image, altering the contrast between structures so that certain details show up more clearly than they otherwise would.
What the NHS says a contrast agent is
In its MRI scan page, dated 11 September 2025, the NHS describes the agent as a contrast medium, given as an injection at the scan appointment, "to help show more detail in the MRI scan pictures". The phrasing is worth pausing on. The agent is not what makes the pictures; it is what helps certain details appear within pictures the scanner was already going to take. The NHS frames it as an optional addition to the appointment rather than the point of the examination itself.
For CT, the NHS page dated 8 November 2023 describes a wider range of routes: the contrast agent may be given as a drink, by injection, or rectally. The same page notes transient sensations that can accompany it, including feeling "hot and flushed" or noticing "a metal taste in your mouth". The NHS describes these as sensations that can accompany the injection.
Angiography: when the agent is the whole point
Angiography sits at one end of a spectrum. Here the contrast agent is not an optional detail enhancer; it is the mechanism of the examination. The NHS angiography page, dated 30 January 2023, states that the agent "highlights your blood vessels, allowing your doctor to see any problems", and the images produced in this way have their own name: angiograms. Blood vessels do not show clearly on a normal X-ray, and the agent makes them stand out as defined structures. In MRI and CT the agent refines an image that already exists conceptually; in angiography the agent produces the visibility that defines the resulting picture.
How the agent reaches the body, by imaging method
The three uses described by the NHS differ in route and purpose, and the table below gathers them in one place.

Contrast agents as regulated medicines: the MHRA position
Gadolinium-based MRI contrast agents have a legal status in the United Kingdom that separates them from ordinary accessories of scanning. A Drug Safety Update published by the MHRA on 14 December 2017 addresses gadolinium-containing contrast agents directly, describing their purpose in the words "Gadolinium-containing contrast agents (GdCAs) are indicated for the enhancement of magnetic resonance imaging (MRI)". The document concerns linear agents, including restrictions on their use and the removal of certain products, and it exists because these agents fall within the regime for regulated medicines rather than outside it. Describing what the document is and what it covers is a factual matter: it is a medicines regulator's safety communication, dated and published on gov.uk, and it treats MRI contrast agents as substances with a pharmaceutical profile that merits formal review.
Information before exposure: a written employer procedure
Whatever the imaging method, the surrounding framework in England is IR(ME)R 2017, the Ionising Radiation (Medical Exposure) Regulations 2017, SI 2017 No. 1322. The Care Quality Commission describes the regulations as providing "a regulatory framework to protect people against the dangers from being exposed to ionising radiation in a healthcare setting", adding that exposures must be individually justified and optimised.
One requirement bears directly on how a contrast-enhanced examination is explained to the person having it. Schedule 2, paragraph 1(i) of IR(ME)R 2017 requires the employer to have written procedures "providing that wherever practicable, and prior to an exposure taking place, the individual to be exposed or their representative is provided with adequate information relating to the benefits and risks associated with the radiation dose from the exposure". This is not a courtesy or a matter of local preference: it is a specified item on a statutory list of procedures the employer must establish. It means that the conversation about benefits and risks before a scan is anchored in regulation, and the wording of any forms used, many of which appear in the glossary, traces back to this framework.

Easily read as one thing
A contrast medium and a radioactive substance are distinct notions. A contrast medium is a substance that changes how tissue or vessels appear in an image, for example the gadolinium injection described by the NHS for MRI. Radioactive substances, by contrast, fall under a separate licensing regime created by IR(ME)R 2017, regulations 4 and 5, which establish a Licensing Authority and a requirement to hold a licence, with operational guidance published by ARSAC in its notes for guidance, published on 17 April 2014 and updated on 14 July 2026.
Feeling an effect and receiving a radiation dose are also separate ideas. The NHS lists transient sensations such as feeling hot and flushed or a metal taste in the mouth with CT contrast, which are bodily sensations associated with the agent. The IR(ME)R framework concerns the radiation dose from the exposure itself, which is the quantity addressed by the written information procedures in Schedule 2 and by the justification and optimisation duties described by the Care Quality Commission.
